Howard Brown Health Center rebrands Triad Health Practice

Howard Brown Health Center (HBHC) Tuesday announced a rebranding of its Triad Health Practice, including a new name and location.
Effective March 25th, Triad Health Practice will become Aris Health, a newly redesigned patient-centered facility located at 3245 N. Halsted in the heart of Chicago's Lakeview neighborhood. HBHC announced last April that Triad would be relocating to the expanded 4,050 square foot space formerly occupied by Northwestern Memorial Physicians Group.
"We are excited about opening Aris Health in the heart of the community to serve patients in an environment completely focused on their care," said Karma Israelsen, interim president and CEO, Howard Brown Health Center.
In addition to adult primary medical and behavioral care, Aris Health will also provide pediatric and family medicine.
"It's important for us to meet the comprehensive health and wellness needs of those we serve. We added pediatrics as part of our core practice offering last year to serve our patients' children; Aris Health and the expanded space allows us to fulfill this need," said Israelsen.
Aris Health will provide care utilizing through the implementation of a complete patient-centered care model, which focuses on ensuring that patients are involved in the care they receive from their medical providers. HBHC has also partnered with Walgreens to house an on-site pharmacy.
"At Aris Health, we are proud to institute a patient-focused model of care in a new and fresh environment that focuses on our patient experience," said HBHC's Medical Director Dr. Magda Houlberg, M.D.
The rebranding and name change comes as part of HBHC's strategic focus on its mission and to maximize the patient-care experience to those the nearly 40-year-old organization currently serves which includes men, women, transgender, seniors, infants, children, and youth. HBHC will also maintain its relationship with Advocate Illinois Masonic Medical Center.
Plans are currently underway to welcome patients into the newly branded facility with a grand opening and ribbon-cutting ceremony scheduled to take place at Aris Health on March 25, 2013 at 8:00 am at 3245 N. Halsted.
