Vital Bridges, the consolidation of Open Hand Chicago, Community Response and The HIV Coalition, helps more than 2,400 people impacted by HIV/AIDS each year to improve their health and build self-sufficiency. By offering case management, food, nutrition counseling, housing assistance and prevention education, at six sites, clients can access high quality, professional help when they need it.
